Practitioners: information is provided to the practitioners that you make appointments with, and other advisors on your care team, as deemed professionally necessary by us your practitioners. Personal information that you share with a practitioner through the course of an appointment or session is secured, held in confidence by that practitioner, and governed by that practitioner’s industry standards or code of ethics.

One of the benefits of Wellevations, LLC is the collaborative nature of the practitioners and the shared treatment plan created by your care team. The least amount of information is distributed across the care team for them to understand how to serve you best. We will restrict access to your information if requested or if you would no longer like us to share your information with a certain Wellevations, LLC practitioners.

If you invite someone to an in person or online session, it sounds obvious, but they have access to the information you share during that session.

Children under eighteen must have guardian approval

Although the internet makes it impossible for us to know the age of online visitors, we make efforts not to collect personally identifiable information from children under the age of eighteen. Please do not send sensitive data (date of birth, credit card numbers, etc.) by email. It is your responsibility to keep any usernames or passwords used on this site, or any of our third party vendors, secret and confidential.

We do our best to keep your data accurate and up to date, to the extent that you provide us with the information we need to do that. If your data changes (like a new email address), then you’re responsible for notifying us of those changes.
